I can't help with the first three but query four, I have some experience
with.

Most blue-eye species that I am aware of are not long lived like
rainbows which normally seem to be able to do about 8 years (or even
longer for the large species). Blue-eyes have done pretty well if they
make it into their fourth year. Most never get that far, and this is the
time of year they usually seem to drop off the perch (at least ones in
aquaria).

My theory as to why this happens is related to metabolism changing with
the season.
I have no proof of this but my blue eyes always chose September to
depart.
Usually the departed fish just look a bit frayed around the edges.
Sometimes I notice that they have gone off their food a day or two
before they die.

Not very helpful but at least you know you are not alone.

4. I have 38 litre tank of P.gertrudae. 6 had been going well for
several months. Last fortnight they have been dropping off one by
one. No symtoms at all. Water is changed 30% weekly.Water tests as
follows;

pH - 7.0
Nitrite - 0
Ammonia - 0
Nitrate - 5ppm
KH - 5 (89.5)
GH - 9 (161.1)
Temperature - 28c (82f)

No idea why they are dying. Tank is lightly planted. Maybe they need
more cover?
