Do you have a cat, and a 1 inch or bigger hole somewheres? They can jump
out and get eaten pretty quickly :)
Or loricariid catfish? They can snuffle deaders up PDQ...
Perhaps you have discovered the fishy Twilight Zone... I have lost fish this
way myself.
